The Davis and Elkins Athletic Training Department would like to inform you that the athletes are now able to obtain the Sport Medicine Forms online.

The Davis & Elkins College online address is www.dewv.edu. Once on the Home Page, click on Athletics on the navigation bar, then click on Links and finally you'll need to click on Sports Medicine Forms.

These forms must be completed and mailed to us by August 1, 2012 at the address listed below:

*Submission & completion of these forms is required for all student athletes. If any one form is incomplete or not returned, the athlete will not participate in his or her team activities. Including, but not limited to, pre-season camp, practice, or game play.
